我国儿科医师只有6万人，平均每1000个儿童只有约0.26个儿科医师，是美国的1/6；近15年我国儿科医师仅仅增加5000人，缺口超过20万。儿科事业发展面临着严峻的挑战。在刚刚闭幕的北大儿科论坛上，浙江大学附属儿童医院赵正言教授建议国家应加大对儿科建设的投入，建立稳定、制度化的费用保障机制，缩小城乡、地区间儿科服务能力、服务质量的差异，更多的儿童基本医疗服务项目由政府买单，加快儿科多元化办医格局。

　　儿科医生工作辛苦，责任大。在医生圈子里，儿科的地位也不太高。医生间交流时，一听到对方是儿科，通常会说，“小儿科的”，一语双关。但家长对儿科医生的要求却不是“小儿科”。来自台湾的一位儿科专家说，台湾地区的儿科医生同样面临着家长的高要求：“如果扎针第一针没进，第二针要是再扎不进去，家长的脚就上来了。”

　　我国每年的新生儿为1600万至1700万，零至18岁儿童近3亿，政府对儿科医疗投入严重不足，没有儿科发展的总体规划和远期目标。赵正言说，我国的儿童床位数为258224张，仅占全国床位数6.4%；我国的儿童医院只有68家，占医院总数的0.52%，比例不足美国1/10。我国的基本医疗保障及医疗保险系统还无法覆盖全体儿童，人均筹资水平较低，儿童医疗保险覆盖率不高，报销比例也不高。我国没有建立分级就诊与转诊制度。社区医院儿科医师严重缺乏，全科医师儿科知识与诊疗经验严重不足，社区医师积极性不高，导致儿童患者不合理流动。

　　在综合医院，儿科的生存更是困难。北京市已经要求二级以上综合医院都设儿科，但所有综合医院儿科的门急诊量加起来与本市的两家儿童专科医院差不多，有些综合医院儿科一天的门诊量不过几十人，日门诊量突破百人次都很困难。综合医院儿科创收少，导致领导不重视，儿科地位不高导致儿科医师收入少，工作强度大导致医生不愿到儿科工作，甚至有些医院招不到儿科医师。记者了解到，有些儿科医生改行干行政，有些儿科医生下海当了医药代表。综合医院的儿科到底如何生存？北大医院儿科主任姜玉武说，综合医院儿科要顾及到“面”，也就是儿科的常见病、多发病；同时要有“点”，依托本院的优势，发展重点专业。北大医院院长刘玉村说，目前北大医院大兴院区已经获批，建成后将提供400张床位用于支持儿科发展，方便南城和北京以南进京看病的患儿。
