市委副书记、市长张祖林昨日主持召开市政府第45次常务会议，审议通过《昆明市县级公立医院综合改革试点实施意见(送审稿)》(以下简称《意见》)，决定于12月1日起正式实施《意见》，在全市10个县(市)区的20家公立医院启动改革试点工作。

　　《意见》提出，改革试点工作将以破除“以药养医”机制为关键环节，在改革医院补偿机制及建立现代医院管理制度等方面取得突破。通过综合改革，有效控制医药费用过快增长，使次均门(急)诊费用和次均住院天数得到降低。在确保县级公立医院收支平衡的同时，力争使县级公立医院医疗总费用增长控制在上年度的14%以内，县域内就诊率提高到90%，患者和居民满意度提高到95%以上，基本实现大病不出县。

　　《意见》明确，在安宁、呈贡、晋宁、富民、宜良、嵩明、石林、禄劝、寻甸9个县(市)区列为国家县级公立医院综合改革试点县的基础上，把东川区确定为市级公立医院综合改革试点县。今年内，10个县(市)区的20家公立医院全面启动改革工作。各县(市)区的改革试点分别是：安宁市的人民医院、中医医院、云南昆钢医院，呈贡区的人民医院，晋宁县的第一人民医院、第二人民医院，富民县的人民医院，宜良县的人民医院、中医医院，嵩明县的人民医院、中医医院，石林县的人民医院、中医医院，禄劝县的人民医院、中医医院，寻甸县的人民医院、中医医院，东川区的第一人民医院、第二人民医院、中医医院。

　　《意见》明确，政府要给予县级公立医院经常性补助的投入，对医院在编在职人员，每年人均补助经费不低于2万元，并随各地财政收入的增长，加大人员经费补助。取消药品加成政策后，医院由此减少的合理收入，通过调整医疗技术服务价格、增加政府投入两条途径予以补偿。所减少的合理收入，按照“取消多少，补偿多少”的原则，由中央、省、市、县(市)区财政给予足额补助；对住院取消“以药补医”所减少的合理收入，按照每人每天增加20元的住院诊察费给予补助，参保人住院补助资金从医疗保险基金中支付。

　　《意见》明确，服务价格的调整与取消药品加成同步进行。对于其他重点突破的工作目标，《意见》要求，各改革试点县要加大人才队伍建设和引进力度，到“十二五”期末，县级公立医院本科以上专业技术人员要达到35%以上。在县级公立医院管理体制改革中，要建立理事会和监事会制度，实行法人治理结构，建立现代医院管理制度。“十二五”期间，全面完成县级医院标准化建设，每个县至少有一所医院达到二级甲等水平。以县级医院为中心，全市要建立10个县域急救机构，并且县乡医疗服务一体化管理率要达到50%以上，
