记者从西安市政府了解到，西安市政府常务会议19日原则通过《西安市“十二五”期间医药卫生体制改革规划暨实施方案》。根据规划，西安将逐步在社区推行“家庭签约医生”模式，为居民提供连续的健康管理服务。
